Registration and Release Form

At Animal Kingdom, before booking or signing up for a drop-in, daycare, hotel, or dog walk, a registration and release form is needed for you to sign. The form will be provided below for you to download and hand-in at the reception desk in Animal KIngdom. All information must be filled out and read carefully.
